We aimed to investigate the influence on the expansion of cutting balloon with different sizes in coronary artery with calcified plaque by finite element analysis.
The finite element models of blade, pad, balloon, calcified plaque (arc 180deg) and artery were generated. First, the balloon model was crimped and compressed to obtain the folded shape and then three sets of blade models and pad models were constrained to the surface of the balloon model. Next, the whole cutting balloon model was assembled in the calcified artery model and pressure was applied to the inner surface of the balloon model. Different sizes of balloon diameter with two types of blade direction (type1: one blade inside plaque and type 2: two blades inside plaque) were considered and simulated. 
The maximum stresses in the plaque models in type2 were greater than those in type 1. However, in contrast the maximum principal tensile stresses occurred in artery models in type1 were larger than those in type 2. These results indicated that type 2 is more effective and safer than type1 associated with the blade direction during cutting balloon inflation. It showed that the risk of dissection is high between the plaque and artery wall.
